PANS & PANDAS: Finding the Underlying Cause Is the Key to Healing

Parents of children with PANS or PANDAS often describe the moment symptoms begin as terrifying and confusing. A child who was functioning well can suddenly become anxious, aggressive, obsessive, fearful, emotionally fragile, or physically unwell overnight. The medical world still has wide gaps in its understanding—but one thing is becoming increasingly clear:

PANS/PANDAS is not random.
There is almost always a trigger.

And finding that trigger is the difference between temporary relief and true healing.

At Whole Brain Holistic Care, this is at the heart of my work: uncovering what is over-activating the immune system, calming the inflammatory response, and getting your child out of “fight or flight” so their brain and body can finally recover.

Understanding PANS and PANDAS

  •  PANDAS is triggered by streptococcal infection.
  •  PANS can be triggered by any immune stressor—viral, bacterial, fungal, parasitic, or toxic.

This means many children diagnosed with PANS are actually dealing with multiple underlying triggers at the same time, creating a storm within the immune and nervous systems.

Symptoms can include:

  •  Sudden onset OCD or tics
  •  Extreme anxiety
  •  Mood swings or rage
  •  Regression (speech, handwriting, toileting)
  •  Sleep disturbance
  •  Restricted eating
  •  Sensory overload
  •  Motor abnormalities
  •  Cognitive fog

These symptoms are not psychological.
They are immune-mediated neurological inflammation.

Why Identifying the Cause Is Crucial

When the immune system is overwhelmed—by infections, toxins, or inflammation—it shifts into a constant fight-or-flight state. In this state, the nervous system cannot regulate emotions, digestion, sleep, or behavior.

Treating only the symptoms—like anxiety, OCD, or aggression—will not bring full recovery.

You must remove what is triggering the immune system.

This becomes much easier when you understand what the trigger is. Every cause has a different treatment strategy:

  •  Viral triggers → antiviral support
  •  Bacterial triggers → targeted antibiotics or herbal antimicrobials
  •  Mold or environmental toxins → detoxification, removal from exposure
  •  Allergies/Mast cell activation → anti-inflammatory and stabilizing protocols
  •  Autoimmune inflammation → immune modulation
  •  Metabolic or mitochondrial issues → repair and support

When the cause is addressed directly, the immune system can calm down.

When the immune system calms down, the brain can heal.

This is the path to lasting improvement—not just temporary symptom relief.

Common Underlying Triggers in PANS/PANDAS

1. Streptococcal Infection

The classic cause of PANDAS.
Rapid strep tests often miss it—titers and throat cultures are critical.

2. Viral Reactivation

Viruses can flare during stress, puberty, illness, or toxin exposure:

  •  EBV
  •  HHV-6
  •  Coxsackie A/B
  •  CMV
  •  Parvovirus
  •  Influenza
  •  Enteroviruses

Viral-driven PANS often presents with aggression, sleep issues, and severe anxiety.

3. Mold & Environmental Toxins

One of the most overlooked causes.
Mycotoxins create:

  •  Chronic inflammation
  •  Immune dysregulation
  •  Neurological symptoms
  •  Worsening OCD and rage
  •  Severe fatigue and sensory issues

If symptoms worsen at home or improve when traveling, this is a major clue.

4. Lyme & Tick-Borne Illness

Borrelia, Bartonella, Babesia, Mycoplasma and Ehrlichia can all trigger PANS.

Bartonella in particular is linked to:

  •  Rage
  •  OCD
  •  Leg pain
  •  Mood instability
  •  Sensory issues

5. Gut Inflammation & Dysbiosis

The gut-immune-brain axis is powerful.
An inflamed gut can create:

  •  Immune activation
  •  Neurotransmitter disruption
  •  Worsening OCD/anxiety
  •  Poor detoxification

6. Autoimmunity & Mast Cell Activation

Many children with PANS have overlapping immune disorders that must be stabilized so the body can heal.

How Treating the Cause Breaks the Fight-or-Flight Cycle

Children with PANS/PANDAS often live in a chronic state of survival.
They are not choosing their behaviors—
their brain is trying to protect them from a perceived threat.

When the true trigger is found and removed:

✔ The immune system stops overreacting

✔ Inflammation goes down

✔ The nervous system shifts out of survival

✔ Speech, emotional regulation, sleep, and behavior improve

✔ The child becomes more themselves

Parents regularly report that “the fog lifts” or “my child comes back.”

The Holistic Path I Use in My Practice

My approach to PANS/PANDAS includes:

✔ Detailed medical history

Onset, patterns, exposures, infections, family autoimmune history.

✔ Comprehensive lab testing

To identify viral, bacterial, environmental, metabolic, or autoimmune triggers.

✔ Targeted treatment

Not generic protocols.
Individualized, based on the identified cause(s).

✔ Calming the immune system

Anti-inflammatory, antiviral, detoxification, immune-modulating, and mitochondrial support.

✔ Restoring nervous system balance

So the child can get out of fight-or-flight and into healing mode.

Final Thoughts: Your Child Can Heal

PANS/PANDAS can feel overwhelming and chaotic—but there is a path forward.

When we identify the root causes and treat them directly, we relieve the burden on the immune system.
When the immune system is calmer, the brain can begin repairing.
And when inflammation goes down, the child you know and love can shine again.

You do not have to navigate this alone.
If you suspect PANS or PANDAS, or you feel like something deeper is going on with your child, Whole Brain Holistic Care is here to help guide you every step of the way.
